Hybrid Role: 1–2 days telehealth (remote) 3–4 days in patients'

We are seeking Nurse Practitioners or Physician Assistants to join
a home-based care team serving patients with chronic and complex
conditions, including kidney disease. Youll work alongside a
Registered Nurse and Social Worker to deliver personalized,
patient-centered care through a mix of in-home visits and
telehealth. Key Responsibilities Perform comprehensive assessments
and annual wellness visits (home & virtual) Develop and manage
individualized treatment plans Prescribe medications and order
diagnostics Coordinate care with physicians, specialists, and care
team members Educate patients and families on disease management
and preventive care Maintain accurate, timely clinical
documentation Requirements Active NP or PA license and board
certification Active DEA license 2 years of direct patient care
experience Ability to practice independently Valid driver's license
